GOLF: To Qualify for handicap play, all 4 players must have an established handicap. If no handicap, indicate Callaway. Please indicate one member as Team Captain.

Maximum recognized differential of 10 strokes in the Handicap flight. No differential in the Callaway flight.

If you have more than 4 players you will be able to add them after this form has been submitted.
